What are your options?

To be eligible for Health First Colorado (Colorado's Medicaid Program), ACC: Access KP, Child Health Plan Plus (CHP+) and the Kaiser Permanente Colorado Bridge Program you must meet certain eligibility requirements.

Health First Colorado (Colorado's Medicaid Program)

  • Low-income Coloradans that qualify.
  • Family size and household income help to determine if you qualify. To see a Health First Colorado income eligibility chart, click here.
  • U.S. citizen or legal permanent resident for at least 5 years, some Health First Colorado exemptions apply. (Refugees, asylees are examples of exemptions.) Effective July 1, 2015 - children and pregnant woman who have been Legal Permanent Residents for less than 5 years may be eligible for Health First Colorado and CHP+.
  • Must be a Colorado resident.

Child Health Plan Plus (CHP+)

  • Income eligible children age 18 and under, pregnant women age 19 and over and NOT eligible for Health First Colorado (Colorado's Medicaid Program).
  • Family size and household income help to determine if you qualify. To see a CHP+ income eligibility chart, click here.
  • U.S. citizen or legal permanent resident for at least 5 years, some Health First Colorado exemptions apply. (Refugees, asylees are examples of exemptions.) Effective July 1, 2015 - children and pregnant woman who have been Legal Permanent Residents for less than 5 years may be eligible for Health First Colorado and CHP+.
  • Must be a Colorado resident.
  • Does not have other health insurance.

ACC: Access KP

  • Health First Colorado (Colorado's Medicaid Program) members who live in Adams, Arapahoe or Douglas counties and want to see a Kaiser Permanente provider. 
  • Must be a part of the Accountable Care Collaborative Program. 
  • Must have a Kaiser Permanente primary care medical provider. 
  • This program doesn't include Medicare Medicaid Program (MMP) members and Medicaid Secondary members. 
     

Colorado Bridge

  • Low-income applicants under the age of 35 years at the time of the effective date of the Kaiser Permanente plan.
  • Family size and household income help to determine if you qualify. To see a Colorado Bridge income eligibility chart, click here.
  • Must live within the Kaiser Foundation Health Plan of Colorado service area based on county zip code, click here to check your zip code.
  • Must not have access to any other public or private health coverage including, but not limited to:
    -  Health First Colorado (Colorado's Medicaid Program)/CHP+
    -  Connect for Health Colorado
    -  Medicare
    -  Job based health coverage
